And while checking your issue more deeply, I caught a small bug in the
rewrite handling in case of CF when a call originates from PSTN also.

Please do:

apt-get update
apt-get install ngcp-templates-ce-kamailio
ngcpcfg apply

So if it's not already there, insert an Inbound Domain Rewrite Rule in
the domain your subscriber, which does the CF, is provisioned.

Before the fix above, the peer rewrite rule was used in your particular
scenario, which was obviously wrong in case of a CF to an internal user.
