William,

You simply need to replace 0 by caller's country code as described here:
http://www.sipwise.com/doc/2.4/spce/ar01s04.html#_inbound_rewrite_rules_for_callee

    Match Pattern: ^0([1-9][0-9]+)$
    Replacement Pattern: ${caller_cc}\1
    Description: National to E.164

or:
    Match Pattern: ^0([1-9][0-9]+)$
    Replacement Pattern: 234\1
    Description: National to E.164

(as Inbound Rewrite Rules for Callee). Then you might need the same rule
for caller to display the caller-id in this format. Remove the rule for
local calls for now (the one with ${caller_cc}${caller_ac}).
Assign this Rewrite Rule Set to domain and you should be OK.

If you still have some problem after this please send the screenshot of
what the Rewrite Rule Set looks like.
