What is the primary reason for using multiple domains as opposed to
accounts within a single domain?

It's not possible, at least not yet, but you can disable multi-domain
support using ignore_auth_realm config.yml setting since version 2.5.
If you don't use multi-domain support that would work indeed. Once
switched on, the domain part of subscriber will be ignored on inbound
calls so you can have your extra domain=IP.
