"To" header is irrelevant for calls, what's important is the Request-URI.

What's the username of the called subscriber? What's the R-URI? If it's
not 511111111, and if parts of it are missing to match the E164 number,
then make sure to create a rewrite rule set to normalize 511111111 to
the full E164 number and assign it to the peer where you receive the
call from.

The /var/log/ngcp/kamailio-proxy.log shows what's happening with your
calls, how your numbers are normalized and will give you hints what's
going wrong.
