The correct command is actually "/etc/init.d/ngcp-rate-o-mat start".

Don't try to start /usr/sbin/rate-o-mat directly, just in case you tried
that, because it takes its config vars from the environment, which are
set by the init script. One other way to start it is:

. /etc/default/ngcp-rate-o-mat # note the dot in front of the command!
/usr/sbin/rate-o-mat

Anyways, this is essentially what the init script does, so just use that.
