Thanks Andreas. That was a bit scary.
I got the number by looking at the last lcr_gw id but this of course doesn't
equal the number of rows. I now have 126.

We're going to check this. The default max number of gateways set in the lcr
module of kamailio is actually 128, so the 145 seems a bit odd.
You should be able to raise it by setting a line like this in
proxy/kamailio.cfg.tt2:

modparam("lcr", "lcr_gw_count", 1024)

Either way, we're going to make it configurable in config.yml and
programatically enforce it in the provisioning interface.
