I don't mean to point out the obvious but I make this mistake all the time
by putting the rule in the wrong place ;)

^(\+)([1-9][0-9]+)$
\2

will strip the '+' in both inbound for caller and inbound for callee. But
Jon needs the log to see exactly if that's where the problem is.
