Is the loadbalancer running? I can reproduce the behaviour by stopping
kamailio-lb. You can check by running:

# ps -ef|grep kamailio/lb/kamailio

If this doesn't show around 30 processes, then try starting the LB:

# /etc/init.d/kamailio-lb start

And then check if the error is gone.
