We don't use freeswitch, so I have no idea what to make out of this 
logs. So let's try it in another way:

What is your exact call flow scenario, what is your domain/subscriber 
setup (which usernames, which domains), which value did you expect in 
which headers and what did you actually get instead? A "ngrep-sip b" of 
a call would also help to see the call flow, where you can point out 
then which headers need to be different for you to work.
