is it the call from trunk to SIP phone? Please check what is selected in
inbound_upn and inbound_npn preferences of the incoming peer (and if
this is where the peer provides the caller-id).

Then check the outbound preferences of your domain:
outbound_from_user
outbound_from_display
If you can't get it to work send us the kamailio-proxy.log.
