Maybe you need an Inbound Rewrite Rule for Callee in orer to strip the +
and if you add it and it still doesn't work it may be worth increasing
the kamailio log level from NOTICE (default in 3.0) to INFO:
ngcp-kamctl proxy fifo debug 2
