what version do you have?
The fix is available in mr3.4.2 since mr3.4.2.8 and in mr3.5.1 since the beginning. Please show your:
dpkg --list ngcp-templates-ce-kamailio | egrep "^ii"
also check if you don't have the proxy.cfg.customtt.tt2 from the older version.

If you are running the current version you should provide me the new trace and kamailio log to check.
FTR: the issue occurred when both settings are set to no:
kamailio.proxy.allow_info_method: no, kamailio.proxy.allow_refer_method: no
