It means kamailio failed to insert record for some reason.
There should be more info in log. DB gone? DB connection gone?
What is inside MySQL logs for those time?

Connection can be closed by inactivity timeout, kamailio failed to
inster record, reconnected and inserted properly, etc...

Hard to say from this side, share steps to reproduce if you have
together with release version.

Please always specify release version you have.
