[Spce-user] Call Routing Verification 500 Internal server error

I posted a fix in

To fix add

use NGCP::Panel::Utils::Form;

to Verify.pm on line 10 
(/usr/share/perl5/NGCP/Panel/Form/CallRouting/Verify.pm)
