As you can see the expected partition mapping for the upgrade is not
present in you deployment.
The upgrade scripts expects something like this.
~# df -h
Filesystem Size Used Avail Use% Mounted on
udev 32G 0 32G 0% /dev
tmpfs 6.3G 1.4M 6.3G 1% /run
/dev/mapper/ngcp-fallback 9.8G 5.3G 4.0G 58% /
tmpfs 32G 33M 32G 1% /dev/shm
tmpfs 5.0M 0 5.0M 0% /run/lock
/dev/mapper/ngcp-root 9.8G 4.7G 4.6G 51% /ngcp-fallback
/dev/sdb2 486M 176K 485M 1% /boot/efi
/dev/mapper/ngcp-data 377G 28G 330G 8% /ngcp-data
tmpfs 6.3G 0 6.3G 0% /run/user/0
With partitions
/dev/mapper/ngcp-root
/dev/mapper/ngcp-fallback
I guess the only options is to re-install.
