Looks like kamailio-proxy is not reachable. Check with
"/etc/init.d/kamailio-proxy status" and "netstat -plen|grep 5062" if
it's really running, and also when starting with
"/etc/init.d/kamailio-proxy start" check the kamailio-proxy.log.
