Actually there are two features you'd need for this:

1) Forward specific (blocked) numbers to a configurable destination.

2) Forward (or block) numbers based on entries in the address book.

Both sound basically interesting and doable. The forward part should be
easy to implement in the kamailio config, an additional forwarding type
(call forward barring) may also be handy.

Reading the call forward numbers from the address book may not be a good
idea, maybe it's better to synchronize the numbers to a dedicated table,
or create usr_preferences entries with them. The detailed implementation
requires some more thoughts, also on how the integration of the
provisioned data with kamailio should work.
