The provider info is the billing contract for the
destination_provider_id column in the CDR, which must therefore be a
valid billing contract id. Do you have a billing contract with id 5? If
not (maybe you deleted it, and if so, how?) then you need to manually
set the rating_status to 'ok' or 'failed' for the CDRs in question. You
must never delete a billing contract from the DB if there are still
unrated CDRs for this contract.
