init and follow rate/interval allow you to charge different at the start and
the following intervals of the call.

Examples:

You want to charge 15 cent as call stablishment and 6 cents per minute to that
destination. You would set something like this:

init rate: 15
init interval: 1
follow rate: 0,1
follow interval 60

This would charge 15 cents the first second and 6 cents if the call reaches 60
(actually 61 because the init interval is 1) seconds. If the call is
lower than that the cost would be 15 cents because the following interval has
not been reached.

In many countries, by law, you need to charge per second. If you'd like to
charge 6 cents/minute but charge per second and charge no matter the duration
is it would be:

init rate: 15,1
init interval: 1
follow rate: 0,1
follow interval 1

For no stablishment fee. Just 6 cents/minute:

init rate: 0,1
init interval: 1
follow rate: 0,1
follow interval 1


Nowadays, the intervals are usually 1 second. In the past there were different
fees for the first minute and the rest of the conversation and the charges
didn't have to be per second as they are now. Even more: To fulfill the
regulations of some countries, the billing precision of the provider changes
from seconds to miliseconds a couple of releases ago.
