Well, yeah, you'd probably need to do some customizations to 
/etc/ngcp-config/templates/etc/kamailio/lb/kamailio.cfg.tt2. See 
http://kamailio.org/docs/modules/3.3.x/modules_k/nathelper.html#idp122472 for 
some flags controlling the NAT detection.
