You could always telnet into the device and run

Terminal monitor
Debug ccsip messages

This will give you some simple sip debugging on the cisco equipment
Remember to turn off debugging when done

Or you could hire me....lol

All mine are in the US also. I'm thinking this is either not matching up or the incoming dial plan like you stated. A Cisco partner did the equipment and install.  He just wants me to provide service to it. I've just never fooled with one and I get the feeling he doesn't have a clue.

I think we have it in the incoming dial plan. I will check.

On the rewrite rule did you do something special just for the uc520 or make changes to the UC?

Did you add your DIDs to the incoming dial plan on the cisco box and assign them to a destination.

Also could be your rewrite rules, the phone system wont by default take E.164
