[Spce-user] Sipwise install, ACK and BYE routing issue. Please help.

Hi all. I'm having a problem with SIP messages ACK and BYE having no effect after setting up sipwise:ce Latest Feature Release – mr3.2.1.
How can I debug this issue? I can see the SIP messages by doing a tcpdump. It's probably an IP address config issue since I have a custom setup. Detailed below.
2 subscribers setup on different IP phones. Both register and I can call both and hear audio on both ends perfectly. But when I hang up one end the other does not hang up. Leaving the suspicion that the BYE does not get routed correctly. I've checked the Kamailio config and SEMS config and both seem correct. Maybe there's a config issue with my kamailio-lb and kamailio-proxy config?
Using Wireshark SIP Flow I can see the SIP ACK and BYE not making it to their destination.
I have a custom setup. I'm using a VPN with a private server in the same subnet as the Sipwise server. Both IP phones can ping the Sipwise server (all ports open).
VPN server - 10.0.0.74
Sipwise server - 10.0.0.127
dhcp for clients - 192.168.7.x
