[Spce-user] 500 Internal server error mr5.5.1

Hi Alex,

I tried all upgrade steps on a VM but with the same results. Just 
filling the form always results in the internal error, no special data 
required.

I think "use NGCP::Panel::Utils::Form;" is missing in Verify.pm (this 
fixed it for me, inserted on line 10)
