Sorry for the delay...
Can you post the output of:
-Physical Server-
1) fdisk -l
2) lvdisplay
3) cat /etc/fstab
-VM- (booted from LIVE-CD, example GRML https://grml.org/)
1) /etc/init.d/lvm2 start (or '/etc/init.d/lvm start', not sure 100%)
2) fdisk -l
3) lvdisplay
4) cat /etc/fstab
